ABSTRACT

Resumo: Este trabalho tem como objetivo analisar os principais padrões de organização das redes municipais de serviços de atenção primária à saúde (APS) e avaliá-los segundo os indicadores de interface entre gestão e gerenciamento local. Trata-se de pesquisa avaliativa que analisou 461 municípios de São Paulo, Brasil, que participaram do Inquérito de Avaliação da Qualidade de Serviços de Atenção Básica (QualiAB) em 2017/2018, classificados segundo a composição dos arranjos organizacionais de 2.472 serviços de APS. Para avaliar os padrões identificados, foram selecionados oito indicadores de gestão e gerenciamento local. Os resultados apontam dois grupos de municípios: homogêneos, com serviços de um mesmo arranjo (43,6%); e heterogêneos, com diferentes arranjos (56,4%). Os grupos foram subdivididos em sete padrões que variaram entre homogêneo-tradicional, homogêneo-Estratégia Saúde da Família, homogêneo-misto e diferentes combinações no grupo heterogêneo. Todos os indicadores apontaram diferenças significativas entre os grupos (p < 0,001), com destaque para o grupo homogêneo-tradicional, com padrão organizacional distante do modelo desejado para uma APS abrangente e resolutiva, enquanto aqueles com unidades de saúde da família (USF), e com unidades básicas com agentes comunitários de saúde e/ou equipes de saúde da família (UBS/USF) demonstraram um padrão mais aproximado desse modelo - com ações de planejamento e avaliação comprometidos com a realidade local e com a qualificação do trabalho. Discute-se a importância das políticas implementadas pela gestão federal e estadual e seu poder de indução na definição do modelo de atenção à saúde na APS dos municípios.


Resumen: El trabajo tiene el objetivo de analizar los principales patrones de organización de las redes municipales de servicios de atención primaria de salud (APS) y evaluarlos conforme los indicadores de interfaz entre la dirección y gestión local. Se trata de una investigación evaluativa que analizó 461 municipios de São Paulo, Brasil, que participaron de la Encuesta de Evaluación de la Calidad de los Servicios de Atención Primaria (QualiAB) en 2017/2018, clasificados según la composición de los arreglos organizativos de 2.472 servicios de APS. Para evaluar los patrones identificados, se seleccionaron ocho indicadores de dirección y gestión local. Los resultados indican dos grupos de municipios: homogéneos, con servicios de un mismo arreglo (43,6%) e heterogéneos, con arreglos diferentes (56,4%). Los grupos se subdividieron en siete patrones que iban desde homogéneo-tradicional, homogéneo-Estrategia de Salud de la Familia, homogéneo-mixto y diferentes combinaciones en el grupo heterogéneo. Todos los indicadores señalaron diferencias significativas entre los grupos (p < 0,001), con destaque para el grupo homogéneo-tradicional, con patrón organizativo alejado del modelo deseado para una APS completa y resolutiva, mientras aquellos con unidades de salud de la familia (USF), y con unidades básicas con agentes comunitarios de salud y/o equipos de salud de la familia (UBS/USF) demostraron un patrón más cercano a este modelo -con acciones de planificación y evaluación comprometidas con la realidad local y con la calificación del trabajo. Se discute la importancia de las políticas implementadas por la gestión federal y la gestión estatal y su poder de inducción para definir el modelo de atención a la salud en la APS de los municipios.


Abstract: This study analyzes the main organization patterns used by primary health care (PHC) services in municipal networks and evaluates them according to indicators of local management-administration interface. Evaluative research analyzed 461 municipalities in São Paulo, Brazil, that participated in the Primary Care Services Quality Assessment Survey (QualiAB) in 2017/2018, classified according to the organizational arrangements composition of 2,472 PHC services. Eight indicators of local management and administration were selected to evaluate the identified patterns. Results indicate two groups of municipalities: homogeneous, with services presenting the same arrangement (43.6%); and heterogeneous, with different arrangements (56.4%). These were subdivided into seven patterns that ranged from homogeneous-traditional, homogeneous-Family Health Strategy, homogeneous-mixed, and different combinations in the heterogeneous group. All indicators showed significant differences between groups (p < 0.001), especially the homogeneous-traditional group, which presented an organizational pattern far from the desired model of a comprehensive and problem-solving PHC. Those integrated with family health units (FHU) and basic health units with community health workers and/or family health teams (BHU/FHU) showed a pattern closer to a comprehensive model - with planning and evaluation actions committed to the local reality and qualification of care. Implementation of federal and state policies are essential for defining the PHC health care model adopted by municipalities.

ABSTRACT

Introducción: la Medicina Nuclear es una especialidad médica que trata o diagnostica diferentes afecciones mediante imágenes de tipo funcional-molecular, a partir del empleo de fuentes no selladas. Un aspecto importante en el desempeño profesional es la adecuada gestión de sustancias que puedan resultar nocivas. Objetivo: fundamentar la estructura conceptual de la competencia gestión de desechos radiactivos. Métodos: se realizó una revisión bibliográfica con un análisis crítico reflexivo, se consideraron documentos normativos de la actividad en Medicina Nuclear, tesis, artículos y libros, publicados a partir del 2018 en español e inglés. La búsqueda fue realizada en las bases de datos SciELO durante el periodo comprendido de enero a mayo del 2023. Las palabras clave utilizadas fueron: competencias, gestión y desechos radiactivos. Fueron revisados 73 textos y se seleccionaron 20 para conformar el texto final. Resultados: se delimitaron cuatro núcleos temáticos: pertinencia de formar competencias profesionales, base teórico-metodológica, base legal y riesgos e implicaciones. Resulta evidente el insuficiente tratamiento teórico-metodológico a la gestión de desechos como actividad de los tecnólogos de Medicina Nuclear. Se propone la definición de la competencia, los problemas contextuales que aborda, ejes procesuales, criterio de desempeño y evidencias requeridas. Conclusiones: el estudio de las competencias profesionales en los tecnólogos de la salud es un campo de notable vigencia encaminado a desarrollar su profesionalización. La evidente relación establecida entre la competencia gestión de desechos radiactivos y la reducción de riesgos y accidentes, conlleva la responsabilidad de formar profesionales preparados para desempeñarse con éxito en la Medicina Nuclear.


Introduction: Nuclear Medicine is a medical specialty that treats or diagnoses different conditions through functional-molecular images, using unsealed sources. An important aspect in professional performance is the proper management of substances that may be harmful. Objective: to support the conceptual structure of the radioactive waste management competence. Methods: a bibliographic review was carried out with a reflective critical analysis, normative documents of the activity in Nuclear Medicine, theses, articles, and books, published since 2018 in Spanish and English, were considered. The search was carried out in the SciELO databases from January to May 2023. The keywords used were competencies, management and radioactive waste. 73 texts were reviewed and 20 were selected to make up the final text. Results: four topic cores were defined: relevance of training professional competencies, theoretical-methodological base, legal base and risks and implications. The insufficient theoretical-methodological treatment of waste management as an activity of Nuclear Medicine technologists is evident. The definition of the competence, the contextual problems it addresses, procedural axes, performance criteria and required evidence are proposed. Conclusions: the study of professional competencies in health technologists is a field of notable validity aimed at developing their professionalization. The evident relationship established between radioactive waste management competence and the reduction of risks and accidents entails the responsibility of training professionals prepared to perform successfully in Nuclear Medicine.

ABSTRACT Objective To assess the agreement between complementary feeding indicators established by the World Health Organization (WHO) and the Ministry of Health (MOH) and to compare the prevalence of these indicators in the first year of a child's life. Methods : This was a cross-sectional study in a cohort of 286 children from Vitória da Conquista, state of Bahia, Brazil; agreement between indicators and comparison between prevalences were analyzed using the Kappa coefficient and McNemar's test; the prevalence of the indicators "introduction of complementary feeding" (ICF), "minimum dietary diversity" (MDD), "minimum meal frequency" (MMF) and "minimum acceptable diet" (MAD) were calculated. Results : Three indicators showed poor agreement, with only one demonstrating moderate agreement; prevalence of WHO indicators was higher than that of the MOH (ICF, 94.3% vs. 20.7%; MDD, 75.2% vs. 50.7%; MMF, 97.2% vs. 44.8%; MAD, 96.8% vs. 26.9%). Conclusion The majority of indicators showed poor agreement and the prevalence of WHO indicators exceeded that of the Ministry of Health.


RESUMEN Objetivo evaluar la concordancia entre indicadores de alimentación complementaria definidos por la Organización Mundial de la Salud (OMS) y el Ministerio de Salud (MS), y comparar la prevalencia entre estos indicadores en niños de un año. Métodos estudio transversal en una cohorte de 286 niños de Vitória da Conquista, Bahía, Brasil; se calculó la prevalencia de "introducción de alimentos complementarios" (IAC), "diversidad dietética mínima" (DMD), "frecuencia mínima de comidas" (FMR) y "dieta mínima aceptable" (DMA); para evaluar la concordancia y comparar prevalencias se utilizó el índice Kappa y la prueba de McNemar. Resultados cuatro indicadores mostraron un acuerdo pobre y sólo uno moderado; las prevalencias fueron mayores según la definición de la OMS (IAC, 94,3% vs 20,7%; DMD, 75,2% vs 50,7%; FMR, 97,2% vs 44,8%; DMA, 96,8% vs 26,9%). Conclusión la mayoría de las concordancias entre los indicadores fueron deficientes, con prevalencias más altas según las definiciones de la OMS.

ABSTRACT

RESUMO Ao final do ano de 2019 o mundo foi surpreendido pela Covid-19, que chegou ao Brasil no início de 2020, fazendo com que o Sistema Único de Saúde (SUS) adotasse estratégias imediatas para atender as necessidades de saúde da população, colocando em evidência todos os obstáculos que o sistema de saúde vinha enfrentando nos últimos anos. O estado de São Paulo não foi diferente do restante do País em relação ao enfrentamento. Este estudo objetivou identificar as principais estratégias adotadas em 5 regiões de saúde de São Paulo, com foco na gestão regional, analisando os processos e práticas adotados para o enfrentamento à pandemia da Covid-19. Tratou-se de um estudo qualitativo, realizado através de estudo de casos múltiplos, com abordagem exploratória, a partir de pesquisa de campo e realização de oficinas regionais. As regiões de saúde deste estudo puderam comprovar a importância da intersetorialidade nas ações de saúde como um todo. Notou-se o grande papel dos municípios nestas ações e a união destes, fortalecendo o papel da regionalização e ampliando a importância da governança em saúde. Ademais, as regiões de saúde saíram fortalecidas porque exerceram seu papel de liderança e organizaram ações junto aos municípios.


ABSTRACT At the end of 2019 the world was surprised by COVID-19, which arrived in Brazil at the beginning of 2020, causing the Unified Health System (SUS) to adopt immediate strategies to meet health needs, as well as putting in check all the obstacles that the health system has been facing in recent years. In the state of São Paulo it was not different from the rest of the country in relation to the confrontation. This study aimed to identify the main strategies adopted in 5 health regions of São Paulo, focusing on regional management, analyzing the processes and practices adopted to face the COVID-19 pandemic. It was a qualitative study, carried out through a multiple case study, with an exploratory approach, based on field research and regional workshops. In this study, the health regions were able to prove the importance of intersectoral actions in the health system. It was noted the great role of municipalities in these actions and their union, strengthening the role of regionalization and expanding the importance of governance in health. In addition, the health regions were strengthened because they exercised their leadership role and organized actions with the municipalities.

El caso de los espacios clínicos odontológicos en Chile, en una primera instancia, deben ser observados como organizaciones altamente complejas y multidimensionales -debido a que dentro de ellas operan y funcionan tres lógicas de funcionamiento, por un lado, la educación formativa de pregrado y posgrado en odontología, la perspectiva de salud producto de las prestaciones que realiza y oferta para el desarrollo de prácticas, aprendizajes y procedimientos odontológicos, por tanto, no tan solo serán espacios organizacionales tradicionales universitarios, sino que más bien serán campos sociales complejo de analizar.


The case of dental clinical spaces in Chile, in the first instance, should be observed as highly complex and multidimensional organizations - because within them operate and function three logics of operation, on the one hand, undergraduate and postgraduate training education in dentistry, the health perspective resulting from the services provided and the offer for the development of dental practices, learning and procedures, therefore, they will not only be traditional university organizational spaces, but rather complex social fields to analyze.

ABSTRACT

Objetivo. Determinar el cumplimiento a las recomendaciones de la Organización Mundial de la Salud (OMS) de las noticias sobre suicidio emitidas por canales de la televisión peruana. Materiales y métodos. Estudio cuantitativo y descriptivo. La unidad de análisis fueron las noticias de suicidio emitidas por ocho canales de televisión peruana durante los años 2020 y 2021. Las noticias se clasificaron en tres categorías: noticias sobre suicidio, intento de suicidio y prevención del suicidio. Se utilizó un instrumento de evaluación compuesto por las recomendaciones de la OMS. Resultados. Se analizaron un total de 126 noticias. Ninguna de estas noticias cumplió en su totalidad con las recomendaciones de la OMS. Las noticias sobre suicidio o intento de suicidio cumplieron con 4 de 13 recomendaciones, siendo la recomendación más acatada la de evitar publicar notas suicidas (97,4%), mientras que la menos cumplida fue la de instruir a la población sobre el suicidio y su prevención, y no difundir mitos (0,9%). Las noticias de prevención de suicidio cumplieron 5 de 7 recomendaciones. Conclusiones. Las noticias sobre suicidio emitidas en la televisión peruana en los años 2020 y 2021 mostraron un bajo cumplimiento a las recomendaciones de la OMS. Los profesionales de la comunicación tienen un rol fundamental en la prevención del suicidio, limitando la información sensible y difundiendo información de ayuda. Es fundamental el conocimiento de estas recomendaciones por los comunicadores, y el trabajo conjunto entre el equipo periodístico y los profesionales de la salud mental en la comunicación de noticias relacionadas con el suicidio.


Objective. To determine if suicide news reports broadcasted by Peruvian television channels comply with the World Health Organization (WHO) recommendations for reporting suicide news. Materials and methods. Quantitative and descriptive study. The unit of analysis was the suicide news broadcasted by eight Peruvian television channels during the years 2020 and 2021. News were classified into three categories: news about suicide, attempted suicide and suicide prevention. We used an evaluation instrument composed of WHO recommendations. Results. A total of 126 news reports were analyzed; but none of these complied fully with the WHO recommendations. The news reports on suicide or attempted suicide complied with 4 of 13 recommendations. Most reports complied with avoiding to publish suicide notes (97.4%); on the other hand, educating about suicide and its prevention, and not disseminating myths was the recommendation least complied with (0.9%). Suicide prevention news reports complied with 5 of 7 recommendations. Conclusions. Suicide news reports on Peruvian television in 2020 and 2021 showed low compliance with WHO recommendations. Communication professionals have a fundamental role in suicide prevention, limiting sensitive information and disseminating helpful information. It is essential for communicators to be aware of these recommendations and for the journalism team and mental health professionals to work together in the communication of news related to suicide.

ABSTRACT

Objetivo: El objetivo general de la investigación fue analizar el cuidado humanizado: Reto para el profesional de enfermería. Método: Se desarrolló desde un enfoque cuantitativo con una metodología descriptiva con diseño no experimental, la cual se apoya en el análisis documental­bibliográfico. Se organizó un proceso investigativo en donde la población de estudio, se basó primordialmente en documentos escritos como tesis, revistas arbitradas y artículos científicos. Resultados: Se planteó la idea de desconstruir el significado de humanizar y reconstruir el concepto de cuidad desde la humanización. En conclusión: Sin duda alguna el profesional de enfermería se enfrenta a grandes retos en la gestión de cuidado humanizado, en un mundo globalizado y en donde se están desarrollando tecnologías que buscan desplazar la acción del ser humano, sin embargo, la calidez y acompañamiento que ofrece este profesional a los pacientes y familiares no podrán ser sustituidos.


Objective: The general objective of the research was to analyze humanized care: Challenge for the nursing professional. Method: It was developed from a quantitative approach with a descriptive methodology with non-experimental design, which is supported by documentary-bibliographic analysis. A research process was organized in which the study population was based primarily on written documents such as theses, peer-reviewed journals and scientific articles. Results: The idea of deconstructing the meaning of humanizing and reconstructing the concept of care from humanization was proposed. In conclusion: Undoubtedly, the nursing professional faces great challenges in the management of humanized care, in a globalized world where technologies are being developed that seek to displace the action of the human being, however, the warmth and accompaniment offered by this professional to patients and family members cannot be replaced.

ABSTRACT

Objetivo: A falta de leitos hospitalares no Brasil é queixa comum entre usuários do Sistema Único de Saúde. Objetivo: Relatar a experiência da construção de um Serviço de Gerenciamento de leitos e apresentar a atuação do enfermeiro como gestor, em prol da visibilidade e fortalecimento da classe de enfermagem. Método: Relato de experiência da implementação da gestão de leitos de um hospital público estadual de médio porte, em um município do interior do estado de São Paulo. Resultado: A partir da implantação houve mudanças no perfil dos indicadores dos setores assistencias, com a utilização dos leitos aproveitados em sua capacidade máxima. Observou-se a diminuição da fila de espera para internação em consequência do acesso oportuno e ordenado à vaga. Conclusão: Pode-se inferir que o gerenciamento de leitos é efetivo e eficiente na gestão hospitalar com resultados operacionais e financeiros satisfatórios e um fator preponderante para a segurança e satisfação dos clientes.(AU)


Objective: The lack of hospital beds in Brazil is a common complaint among users of the Unified Health System. Objective: To report the experience of the construction of a Bed Management Service and to present the nurse's role as manager, for the visibility and strengthening of the nursing class. Method: Experience report of the implementation of bed management in a public hospital of medium size, in a city in the interior of the state of São Paulo. Result: From the implementation there were changes in the profile of the indicators of the care sectors, with the use of beds used to their maximum capacity. A reduction in the waiting list for hospitalization was observed as a result of the timely and orderly access to vacancies. Conclusion: It can be inferred that the management of beds is effective and efficient in hospital management with satisfactory operational and financial results and a preponderant factor for the customers' safety and satisfaction.(AU)


Objetivo: La falta de camas hospitalarias en Brasil es una queja común entre los usuarios del Sistema Único de Salud. Objetivo: Relatar la experiencia de la construcción de un Servicio de Gestión de camas y presentar la actuación de la enfermera como gestora, para la visibilidad y fortalecimiento de la clase de enfermería. Método: Relato de experiência da implementação da gestão de lechos de um hospital público estadual de médio porte, em um município do interior do estado de São Paulo. Resultado: A partir da implementação houve mudanças no perfil dos indicadores dos setores assistência, com o uso de camas utilizadas ao seu máximo de capacidade. Observou-se a diminuição da fila de espera para internação em consequência do acesso oportuno e ordenado à vaga. Conclusão: É possível inferir que a gestão de camas é eficaz e eficiente na gestão hospitalar com resultados operacionais e financeiros satisfatórios e um factor preponderante para a segurança e satisfação dos clientes.(AU)

ABSTRACT

Introduction: As reported by WHO, hypertension is the third ‘killer’ disease, accounting for one in every eight deaths worldwide. Many modifiable and non-modifiable risk factors are found to be associated with the disease. The present study aims to determine the role of dietary management and physical activity in the control of hypertension in a given population. Material and Methods: A cross-sectional study was conducted that. Study included all hypertensive patients (290) from the field practice area of a tertiary care institute. A self-constructed, pretested questionnaire was used to enquire about the socio- demographic factors and other risk factors associated with hypertension. The data was analyzed using SPSS 23 and valid conclusions were drawn by using the chi-square test. Results: Out of 290 patients, 204 (70.3%) were females and 86 (29.7%) were males. Univariate analysis identified decreased salt intake, decreased fatty food consumption, physical activity, stress management, and spiritual help as highly significant factors affecting hypertension. On multivariate analysis, physical activity, stress management and spiritual help were found to be significant. Conclusion: The study showed that physical activity, stress management and spiritual help were significantly associated with hypertension control among the elderly. Measures should be taken to create awareness about lifestyle modifications in the control of hypertension.

ABSTRACT

Introduction: In developing countries, reproductive tract infection among women commonly goes undiagnosed and their sequel causes various complications. Objectives: To find out the prevalence of RTI/STI through a syndromic case approach and its correlates among women of reproductive age group. Material and Methods: A community-based cross-sectional study was carried out among 500 eligible married women residing in urban slums of Agra by using WHO Syndromic case approch for diagnosis of RTIs/STIs. Results: The overall prevalence of RTI/STD was found to be 32.80% where vaginal discharge (22.60%) and lower abdominal pain (8.20%) were the most common syndromes. On multivariate analysis, religion, caste, type of absorbent used during menses, and history of IUD use were found to be significantly significant. Conclusion: The result of this study highlights the high prevalence and potential risk factors that contribute to the occurrence of RTI. It also emphasizes the need of training and education session of the participants to identify the early symptoms of RTI.

ABSTRACT

Introduction: The knowledge of the effect of sociodemographic and disease-related factors on health-related quality of life (HRQoL) among people living with HIV/AIDS (PLWHA) is important for planning comprehensive health-care services for them. Material and Methods: Two hundred PLWHA on antiretroviral therapy (ART) volunteered to complete a self-reported World Health Organization’s Quality of Life?HIV brief questionnaire (WHOQoL-HIV-BREF) that examines six domains each with four items (physical, psychological, level of independence, social, environmental, and spiritual) with 25 facets and additional 5 facets specific to PLWHA (symptoms of HIV, social inclusion, forgiveness, worries about the future, and death and dying). Results: Only 135 questionnaires from 73 (53.3%) men and 63 (46.7%) women (male: female – 1.14:1) aged 20–82 years(mean ± standard deviation: 42.9 ± 10.5 years) were found complete. Eighty-five (63%) individuals were aged 41–60 years, 78 (57.8%) individuals were matriculates, graduates, or postgraduates, 76 (56.3%) respondents were married, and 38 (28.1%) were widows/widowers. Staying-alone workers comprised 43 (31.8%) individuals. Overall health and HRQoL were rated satisfactory (n = 85.2%) and good/very good (n = 74.8%) by a significantly greater number of individuals (P = 0.001). Pearson’s Chi-squared test showed no statistically significant (P > ?) associations between good HRQoL and variables such as age >40 years, gender, education, marital status, duration of disease, disclosure of serostatus to family, ART for >3 years, and CD4 >200 cells/mL. Conclusion: Regular ART can result in adequate control of immunosuppression and no comorbidities in a majority of PLWHA, family and social acceptance, and financial security can result in overall good HRQoL in all six domains within the WHOQoL-HIV-BREF. The study is limited by its cross-sectional study design and small sample size.

Se sistematizan algunas iniciativas y experiencias de organización comunitaria, en proceso de construcción de territorialidad como lo son las de los participantes de la "población moradora", inmersa en los escenarios del despojo, provocado por las alianzas público-privadas del urbanismo neoliberal en el Valle de Aburrá, que al lograr convertirse en sujetos organizativos consolidan apuestas por el trabajo colectivo de transformación social. La reflexión en modo IAP da cuenta de la forma como en medio de las adversas problemáticas de su mundo de la vida, por medio del trabajo con herramientas formativas, la "población moradora" se constituye como subjetividad erguida y de resistencia, en permanente construcción de un proyecto de incidencia política.


In the midst of the advance of the urban intervention of the alliances between the officialdom and the financial guilds for the benefit of the private sector, which took place in the first two decades of the 21st century in the city of Medellín, some considerations are made on the contextual conditions in which the phenomenon of resistance of the population named dweller in the city of Medellín and the metropolitan area emerges. Thus, some initiatives and experiences of community organization are systematized, whose social subjects overcome the bets of collective work, in the midst of the adversities of the problems of their world of life of the subjectivity that lives from the dispossession by urbanism.

El análisis de la relación entre movimientos sociales, comunicación, tecnologías mediáticas y medios digitales suele concentrarse en los alcances y en lo aparentemente determinantes que estos últimos resultan ser para la acción política. El objetivo de este artículo es describir cómo los usos y las apropiaciones de la tecnologías mediáticas y de los medios alternativos digitales son la expresión de la organización, de la lucha y de la negociación política del movimiento campesino del Magdalena Medio colombiano y no, como pudiera creerse, un factor que determina su potencia. Basado en 19 entrevistas a líderes del movimiento campesino, se concluye que la comunicación mediada por tecnologías mediáticas y medios digitales en el movimiento campesino del Magdalena Medio expresa el carácter horizontal y los principios políticos que rigen al movimiento campesino y no lo que lo dota de potencialidad.


The analysis of the relationship between social movements, communication, media technologies and digital media usually concentrates on the scope and apparent determinants of the latter for political action. The objective of this article is to describe how the uses and appropriations of media technologies and digital alternative media are the expression of the organization, struggle and political negotiation of the peasant movement of the Magdalena Medio region of Colombia and not, as might be thought, a factor that determines its power. Based on 19 interviews with leaders of the peasant movement, it is concluded that communication mediated by media technologies and digital media in the peasant movement of Magdalena Medio expresses the horizontal character and political principles that govern the peasant movement and not what gives it its potential.

RESUMEN Objetivo: Describir el modelo de gestión del cuidado de enfermería en servicios hospitalarios. Metodología: La metodología desarrollada en este estudio se basó en la perspectiva cuantitativa, utilizando tipologías documental-bibliográficas que ayudan a establecer el análisis del objeto de investigación. El material abordado se conformó por tesis de grado, investigaciones científicas, trabajos arbitrados y, con ello, descubrir características y relaciones entre los elementos. En conclusión: El rol de enfermería en el desarrollo de los modelos de gestión del cuidado en los servicios hospitalarios, es de gran importancia ya que establecen los mecanismos de comunicación para su implementación en conjunto con el resto del equipo de salud, que hace vida en los centros hospitalarios.


ABSTRACT Objective: To describe the nursing care management model in hospital services. Methodology: The methodology developed in this study is based on the quantitative perspective using documentary-bibliographic typologies that help to establish the analysis of the research object. The material approached consisted of degree theses, scientific research, refereed works and thus discovering characteristics and relationships between the elements. In conclusion: The role of nursing in the development of care management models in hospital services is of great importance since it is necessary to establish the communication mechanisms for its implementation together with the rest of the health team that works in the hospital centers.

ABSTRACT

Introduction: Medical quality of life (QoL) is an international object of study. Brazilian Plastic Surgeons QoL hasn`t yet been assessed, with few studies to perform any evaluation on the subject or estimate variables involved. Methods: This is a transversal, non-comparative, non-experimental, contemporary and of full selection study. An on-line research was performed between July and December in 2021, through the WHOQOL-bref questionnaire application, added with social-demographic information. The population will be composed of plastic surgery residents and surgeons associated with the Brazilian Society of Plastic Surgery (SBCP). Results: We obtained 168 answers via Google Forms®, which allowed us to determine our population through social- demographic questionnaire and make associations with the WHOQOL-bref findings. Conclusion: The current study identified correlations between sociodemographics and quality of life of the Brazilian plastic surgeon. We bring forward descriptive data on a subpopulation of responders inside the SBCP, which achieved a score positively higher than 50% in all of the questionnaire domains.


Introdução: A qualidade de vida (QV) dos médicos é objeto de atenção e de estudo internacionalmente. A QV dos cirurgiões plásticos do Brasil ainda não foi avaliada e não existem descrições das nuances e variáveis que estão mais ou menos relacionadas com a QV dos mesmos. Método: Trata-se de um estudo transversal, de seleção completa, não comparado, não experimental, contemporâneo. Pesquisa foi realizada on-line, entre julho e dezembro de 2021, por meio da aplicação do questionário WHOQOL-bref aos médicos residentes da especialização em cirurgia plástica e médicos cirurgiões plásticos associados à Sociedade Brasileira de Cirurgia Plástica (SBCP). As características sociodemográficas e ocupacionais serão relacionadas à QV. Resultados: Obtivemos 168 respostas dos formulários preenchidos via Google Forms®, o que possibilitou determinar a população estudada pelo questionário sociodemográfico e realizar as associações com os achados do WHOQOL-bref. Conclusão: O presente estudo identificou associação de fatores sociodemográficos à qualidade de vida do cirurgião plástico brasileiro. Delimitamos uma população específica dentro dos participantes da SBCP, a qual pontuou acima de 50% positivamente em todos os domínios do questionário.

ABSTRACT

Under the current practice in organizing surgical services, proportionate representation of disciplines is provided in the curricular planning and in corresponding clinical functions. This is based on the level of competence expected by the end of training period. The disciplines as a “whole” are placed in general surgery or super specialty. The system of vertical arrangement has some serious concerns. Paradoxically, patients with diseases of simple and routine nature of discipline categorized as super specialty are neglected. Super specialist is unable to attend on account of preoccupation with serious challenging problems. The general surgeon hesitates because of privileging issues, fear of allegations of negligence and litigation. The system of vertical division is based on premise that some disciplines deal with complex procedures and others with only simple and routine nature. This premise is incorrect. Each discipline is a mix of simple and complex cases requiring specialized treatment. Alternate modified organization of surgical service is proposed. Activities of all disciplines are scrutinized according to the level of expected competence by the end of training. Categorization is shifted from the “discipline' to “activities.” Criteria applied for classification of activities are as follows: on completion, the trainee is capable to assume full responsibility-category 1; has gained sufficient experience-category 2; and is conversant with broad understanding of management-category 3. Activities of category 1 from all disciplines are assigned to general surgery and those of category 3 from all disciplines are assigned to respective super specialty. Those in the middle, comprising difficult cases but not requiring specialized training or heavy inputs in equipment, are in category 2. They are assigned to general surgery as additional/optional items, or super specialty, guided by local factors. The scope and practice of general surgery are broadened with a shift from “residual” to “comprehensive” discipline. Advantages, concerns, collateral issues of horizontal distribution of activities, its positive impact on research and education are discussed. It is concluded that the proposed organization of surgical services is a rational, logical, and practical strategy for good-quality surgical care in the society. The super specialists need to be convinced that “taking load off” is good for the specialty.

Introducción. La Organización Mundial de la Salud (OMS) recomienda el uso de tablas de referencia para monitorear el crecimiento y estado nutricional de niños, niñas y adolescentes. El peso corporal, la talla y el índice de masa corporal (IMC) son las variables más utilizadas. El presente trabajo tiene como objetivos estimar los percentiles de peso, talla e IMC de escolares (2009-2011) residentes en el departamento San Rafael (Mendoza) y compararlos con la referencia internacional de la Organización Mundial de la Salud, a fin de establecer su pertinencia para la evaluación del crecimiento y estado nutricional de dicha población. Población y métodos. Se realizó un estudio antropométrico transversal en 3448 escolares de entre 4,00 y 13,49 años de edad. Se utilizó el programa LMS ChartMarker Pro para calcular los valores percentilares de peso/edad, talla/edad e IMC/edad, por sexo y edad, y se compararon con las curvas de la OMS. Además, se calcularon diferencias porcentuales (D%) para estimar las diferencias y su significación estadística mediante prueba de Wilcoxon. Resultados. La población de San Rafael mostró, en varones y mujeres, valores percentilares superiores de peso e IMC (D% ≈7 % y 9 %, respectivamente), y menores de talla (D% ≈0,8 %) que los de la OMS (p <0,05). Conclusión. Las diferencias encontradas alertan sobre el empleo de la referencia OMS en la población escolar de San Rafael, ya que sobreestimaría las prevalencias de sobrepeso, obesidad y desnutrición crónica, y subestimaría la de desnutrición aguda y global. Esta situación resalta la importancia de contar con una referencia local.


Introduction. The World Health Organization (WHO) recommends the use of reference tables to monitor the growth pattern and nutritional status of children and adolescents. Body mass index (BMI), weight, and height are the most commonly used variables. The objective of this study was to estimate the BMI, weight, and height percentiles for school-aged children (2009-2011) living in the department of San Rafael (Mendoza) and compare them to the international World Health Organization reference to establish their relevance for the evaluation of the growth pattern and nutritional status of this population. Population and methods. A cross-sectional anthropometric study was conducted in 3448 school-aged children aged 4.00 to 13.49 years. The LMS ChartMarker Pro software was used to estimate the BMI- for-age, weight-for-age, and height-for-age percentiles, by sex and age, and they were compared with the WHO curves. Besides, percentage differences (%D) were calculated to estimate the differences and their statistical significance using the Wilcoxon test. Results. The population of boys and girls in San Rafael showed higher weight and BMI (%D ≈ 7% and 9%, respectively) percentiles, and lower height (%D ≈ 0.8%) values than WHO reference (p < 0.05). Conclusion. The differences found warn about the use of the WHO reference in the school-aged population of San Rafael since it would overestimate the prevalence of overweight, obesity, and chronic malnutrition and underestimate the prevalence of acute and global malnutrition. This situation highlights the importance of having a local reference resource

Background: During pregnancy, women undergo various physiological changes and also suffer from some common illnesses like morning sickness, which may increase the prescription and use of drugs. Thalidomide tragedy of 1960 has created the concern regarding the use of medications during the pregnancy. Prescriber should be very vigilant, while prescribing the drugs for pregnant women because health of mother and fetus may be at risk. To prevent the morbidity and mortality of the mother and the fetus, safer and effective drugs have to be selected with utmost care. Aims and Objectives: Main objectives of our study were to determine the drugs used during complicated pregnancies and to assess the safety of drugs prescribed using the World Health Organization prescription indicators. Materials and Methods: After getting approval from the Institutional Ethics Committee, this cross-sectional study was carried out in the obstetric department of tertiary care teaching hospital for a period of 3 months. Case sheets of pregnant women with complications were collected and the details regarding demography and complete prescription were collected in a predesigned proforma. The data collected were analyzed using descriptive statistics. Results: Among 160 pregnant women, 71% were multigravida and most of the participants belonged to age group of 20–30 years 84% (134). Disease pattern among pregnant women in our study revealed that common illnesses complicating pregnancy was anemia (51%), followed by pregnancy-induced hypertension, oligohydramnios, and gestational diabetes mellitus. The study showed that hematinics, calcium supplements, and protein powders were the class of medications used in most of the prescriptions, and then by antibiotics, antiemetic, and tocolytics agents. Category A of food and drug administration pregnancy drug category was used more, constituting about (45.12%). No drugs from Category X and D were prescribed to pregnant women and 45% of drugs belonged to generic drug. Conclusion: Anemia was the most common condition, complicating pregnancy and common class of drug used were hematinics and protein supplements. Category A drugs, which were the safest class, were prescribed more in our study. No drugs from Categories D and X were used. This indicates that prescribers are well aware of safer drugs in pregnancy.

Resumo O estudo analisa a elaboração de respostas para o enfrentamento da pandemia da COVID-19 na América Latina. Trata-se de um estudo descritivo com base em análise de documentos, dados e medidas políticas adotadas ou anunciadas entre março e dezembro de 2020 em 14 países latino-americanos. A análise compreendeu a apreciação de teor, conteúdo e abrangência das medidas políticas para contenção e mitigação sanitárias e para a reorganização dos serviços de saúde identificadas nos sites governamentais. Complementarmente, incluiu-se indicadores quantitativos demográficos e relativos à situação epidemiológica e resultado do Sringency index. No geral, as respostas dos países latino-americanos foram heterogêneas, apesar de multisetoriais, caracterizando a complexidade e diversidade decisória frente à pandemia. Conclui-se que ainda há muito a refletir sobre as consequências das fragilidades normativas para a consecução das necessidades multidimensionais durante crises sanitárias.


Abstract The study analyzes the development of responses to address the COVID-19 pandemic in Latin America. It is a descriptive study based on an analysis of documents, data, and policy measures adopted or announced between March and December 2020 in 14 Latin American countries. The analysis included assessment of the content, tenor, and scope of policy measures for containment and mitigation, health care, and reorganization of health services identified on government websites. In addition, quantitative demographic indicators were included, as well as those related to the epidemiological situation and the result of the Stringency index. In general, the responses of Latin American countries were heterogeneous, albeit multisectoral, characterizing the complexity and diversity of decision making when confronting a pandemic. The conclusion drawn is that there is still a great deal to reflect upon with respect to the consequences of regulatory weaknesses for the achievement of multidimensional demands during health crises.
